  1. Surgeon fee

    What the surgeon charges for performing the operation. Some quotes give this figure alone, so check whether the rest of the bill is in the same number.

  2. Anesthesia

    The fee for the anesthesia and for the person who gives it — an anesthesiologist or a nurse anesthetist. It can be billed by a separate provider, so a quote may leave it out.

  4. Implants or materials

  5. Pre-operative labs and tests

    Blood tests and other checks a surgeon may ask for before an operation. A laboratory may bill for these rather than the practice.
