Who weight-loss surgery is for
The American Society for Metabolic and Bariatric Surgery says surgery is recommended at a body mass index of 35 or higher regardless of other conditions, and for people with type 2 diabetes at 30 or higher, and should be considered at 30 to 34.9 when non-surgical methods have not produced substantial or durable results. (ASMBS: is metabolic and bariatric surgery right for you?, no page date.) Whether an operation is right for a person is a decision for that person and a clinician.

Will insurance or Medicare cover it?
A self-pay package is what a program charges when insurance does not pay. The National Institute of Diabetes and Digestive and Kidney Diseases says Medicare and some Medicaid programs may cover the major types of weight-loss surgery when certain criteria are met, and that some insurers require approved surgeons and facilities or a medically supervised weight-loss program first.
